Healthcare in Spain for Retirees

Spain is known for having one of the best healthcare systems in the world—and it’s one of the biggest reasons retirees from the UK, USA, and across Europe choose to settle here. Whether you access the public system or take out private insurance, healthcare in Spain is high-quality, accessible, and far more affordable than in many other countries.

Public vs Private Healthcare

Spain has a universal public healthcare system (Sistema Nacional de Salud), which is free or low-cost for residents who contribute to social security. Many expats, especially retirees on certain visas, also choose private health insurance for quicker access and English-speaking doctors.

Accessing Public Healthcare

Retirees may qualify for public healthcare in one of two ways:

  • EU Citizens with an S1 Form: British retirees receiving a state pension can apply for the S1 form from the NHS, which transfers healthcare costs to the UK while allowing access to Spain’s public system.
  • Residents Who Contribute to Social Security: This includes those employed in Spain or self-employed expats who voluntarily contribute.

Private Health Insurance for Retirees

If you apply for a Non-Lucrative Visa, you’ll need private health insurance to be approved. Requirements include:

  • Full coverage with no co-payments
  • No waiting periods
  • Coverage for pre-existing conditions
  • Policy from an authorised Spanish provider

English-Speaking Doctors and Clinics

In areas with large expat populations such as the Costa Blanca, Costa del Sol, and Balearic Islands, many clinics and hospitals have English-speaking staff. You can also find international hospitals that cater specifically to expats.

Cost of Medication

Prescription drugs in Spain are significantly cheaper than in the USA and UK. A typical prescription co-payment ranges from €2 to €20 depending on the medicine and your age.

Planning Tip:
If you’re over 75 or have pre-existing conditions, finding the right private policy may take more time. Look for insurers that specialise in expat policies and long-term visas.
